  • 1. HUSBAND NAME & AGE. 2. WIFE NAME & AGE 3. ADDRESS, CITY, STATE & PINCODE 4. PHONE NUMBER AND MOBILE NUMBER
  • As soon as we receive your above details, you will be issued a Registration No. and further information will be communicated to you.

PLAN YOUR FIRST VISIT

  • You will be asked to visit the centre on the 2nd Day of your period (Counting the start day of your period as FIRST DAY)
  • Incase if your periods are irregular or they have stopped, kindly call at MED AGE Infertility Centres No 0194-2493777
  • You will have to visit the centre at 8:30 – 9:30 am after having a breakfast; your first day at the centre would be TESTING and CONSULTATION part.
  • Next day when your reports come, there will be REPORT DISCUSSION followed by treatment plan. On this day you will be told when to visit next .
